How do I get copies of my medical records?
There are two ways to obtain your medical records. You may call the Health Information Management (HIM) department at 423-837-3420 and follow the instructions, or you may visit the HIM office in the hospital. If you visit you will need to bring your government-issued photo identification and sign a release. You will then receive your records within 48 hours, excluding weekends and holidays.
